By uploading this batch of employees to be screened by GHRR, the undersigned certifies the following: (1) that I have already obtained a written authorization from each and every employee listed in the batch authorizing the above-referenced employer to obtain one or more consumer reports on the employee for the duration of that employee’s employment with the above-referenced employer, (2) that a stand-alone clear and conspicuous written disclosure has already been provided to each and every employee notifying him/her that one or more consumer reports may be obtained at any time during the period of employment, (3) that these consumer reports will be obtained and used for employment purposes only, (4) that the appropriate adverse action process will be followed, where applicable, and (5) that the above-referenced employer will not use any information contained in the report in violation of any applicable Federal or State equal employment opportunity law or regulation.
